Financial Aid

Financial aid forms are available through your myCCC student portal. Use the descriptions below to find the form that best matches what you need.

Some forms should only be completed when requested by Financial Aid. If you are not sure which form to use, contact the Financial Aid Office before submitting one.

• Ability to Benefit: Use this form to document how you meet the federal education requirement for financial aid, such as having a high school diploma, GED or other state-recognized equivalent, or completing an approved homeschool education. You will need to select the option that applies to you and provide supporting documentation.

• Bookstore Voucher Opt-Out: Use this form if you do not want to use your financial aid money to get your textbooks through the bookstore voucher available to you.
• Cancellation: Use this form to request the cancellation of some or all of your financial aid.
• Comet Promise Application: Use this form to apply for the Comet Promise, CCC’s tuition support program for eligible students. Learn More about the Comet Promise Program 
• Consortium Agreement: Use this form if CCC is your home school and you are also attending another college. A consortium agreement may allow eligible credits at the other school to be considered when CCC determines your financial aid.
• Cost of Attendance Budget Appeal: Use this form to ask Financial Aid to review your Cost of Attendance if your current budget and finances have worsened or are different than the information used for your Free Application for Federal Student Aid (FAFSA).
• Dependent Verification Worksheet: Complete this form if you are a dependent student and Financial Aid asks you to provide information to verify the information provided on the FAFSA.
• Federal Work Study Application: Use this portal to apply for consideration for Federal Work-Study employment. Learn more about Federal Work-Study
• Financial Aid Information Release: Use this form to give CCC Financial Aid permission to release your financial aid information to another person or organization.
• Identity Form: Use this form when Financial Aid asks you to provide documentation to verify your identity. This form is not available online. Contact Financial Aid to obtain this form. [In person?]
• Independent Verification Worksheet: Complete this form if you are an independent student and Financial Aid asks you to provide information to verify your FAFSA.
• Loan Change Request: Use this form to request a change to your student loan. This includes increasing, decreasing, or canceling your loan.
• Parent Refusal to Complete the FAFSA: Use this form when a parent refuses to provide information for a dependent student’s FAFSA. Financial Aid will use the information provided to determine whether the student may qualify for limited federal student loan eligibility.
• Proof of Dependent Request: Use this form when Financial Aid asks you to provide information or documentation about your dependent(s).
• Request for Dependency Override: Use this form to request a review of your dependency status when unusual circumstances prevent you from providing parent information on your FAFSA.
• Satisfactory Academic Progress Appeal: Use this form to appeal a Financial Aid Suspension when circumstances affected your ability to meet CCC’s Satisfactory Academic Progress requirements.
• Senior Citizen Tuition Waiver: Use this form to request the tuition waiver available to eligible senior citizens.
• Special Conditions Appeal: Use this form to ask Financial Aid to review changes in your or your family’s financial circumstances that may not be reflected on your FAFSA.
• Student Authorization to Release Information – FERPA Release: Use this form to give CCC permission to release specified information from your student education record to another person.
• Tuition Assistance Grant for CCC Adult ED & GED Grads: Use this form to apply for tuition assistance available to eligible graduates of CCC’s Adult Education or GED program.
• Unaccompanied or Homeless Youth Verification: Use this form when Financial Aid asks you to provide information related to your status as an unaccompanied youth who is homeless or at risk of homelessness.
• Unusual Enrollment Verification: Use this form when Financial Aid asks you to provide information about your enrollment history at multiple colleges or universities.
