Documentation Guidelines

Documentation helps Disability Resources understand how your disability may affect you in an academic setting and determine what accommodations are appropriate and reasonable. You do not need to have documentation in hand to start the process, submit your request first and our team will work with you from there.

What Counts as Documentation?

Documentation should come from a qualified professional familiar with your condition, such as a:

  • Medical provider
  • Psychologist or psychiatrist
  • Licensed counselor or therapist
  • Educational professional (For example, evaluation reports from a school)

What Should Documentation Include?

When possible, documentation should include:

  • A description of the condition or diagnosis
  • How the condition affects you in an academic environment
  • Relevant functional limitations — for example, difficulty concentrating, reading, mobility, or processing information
  • Recommended accommodations, if available

Not all documentation will include everything on this list. DR will review what you provide and follow up if additional information is needed.

What If You Do Not Have Documentation?

You can still request accommodations even if you don’t have documentation.

After you submit a request:

  • DR will meet with you to learn more about your needs
  • We may discuss options for obtaining documentation if needed
  • In some cases, temporary accommodations may be considered while documentation is being gathered

Our goal is to reduce barriers, not create them.

Previously Used Accommodations

If you had an IEP or 504 Plan in high school, or accommodations at another college, share that information when you apply. It can help guide the conversation, though additional documentation may still be needed.

Confidentiality

All documentation is kept confidential and is used only to determine appropriate accommodations. Information is shared only as necessary to support your approved accommodations.
